*These will be accepted later, yet it is easier to touch base with teachers if this form is filled out before school is out.

IT IS YOUR RESPONSIBILITY TO COLLECT THE SEALED AND SIGNED ENVELOPE FROM YOUR TEACHER AND RETURN IT TO COACH TARLTON'S BOX AT THE FRONT OFFICE OR YOU MAY MAIL IT.

Thank you for your assistance in evaluating potential squad members for the 2008-2009 cheerleading season. Please rate the student as indicated; upon completion, please return this form directly to the student in the envelope provided. The envelope should be SEALED AND SIGNED. If you would prefer returning or mailing (if you are not a teacher at Myers Park) this form to me directly you may place it in my box at the front office (labeled: Cheerleading/Yasmiin Tarlton) no later than June 10th. Tryouts are not until the end of July, yet in an effort to make it easier for students to contact teachers I would like to get these back as soon as possible. All responses are confidential.
